My Care, My Choice: New Tool for Dually Eligible Individuals

 

Illinois offers multiple options for dually eligible individuals to receive healthcare coverage, ranging from integrated care options to traditional Medicare and Medicaid, that are often confusing to understand. My Care, My Choice, an online decision support tool by the National Council on Aging, assists dually eligible individuals (those with Medicare and Medicaid) to better understand their options and identify the best integrated care choice for them. How the Tool Works

 

My Care, My Choice is designed for dually eligible individuals, caregivers, benefit counselors, and Senior Health Insurance Program (SHIP) counselors. The website includes information on the basics of Medicare, Medicaid, and all integrated care options in Illinois. The tool then asks a series of questions about the dually eligible individual’s needs and preferences when it comes to their healthcare coverage. The questions focus on providers and services the individual needs. Once the individual answers all the questions, they will receive a personalized list of coverage options with the one that meets their needs topping the list. 

  

Please note, while individuals can search and read about different plans under each care option, they are not able to enroll directly via the My Care, My Choice website. The website directs individuals to the appropriate website or phone number to enroll in the plan they choose. Individuals can always reach out to their local SHIP counselor (800-252-8966) for assistance in enrolling in a plan.

 

This tool is available in Illinois, Michigan, Ohio, and California.
