Medicare Data Breach Notices and Replacement Cards
The Centers for Medicare and Medicaid Services (CMS) recently announced that 103,000 Medicare beneficiaries were impacted by the creation of unauthorized and fraudulent Medicare.gov accounts using personal information obtained by external and unknown sources. The creation of the unauthorized accounts happened between 2023 and 2025 using personal information that included Medicare Beneficiary Identifiers (MBIs), effective dates, last names, date of births, and zip codes.
Once aware of the incident, CMS immediately deactivated the fraudulent accounts and began monitoring claims data for any suspicious activity. In addition, the impacted beneficiaries were mailed letters explaining the data incident and informing them that that they will be receiving new Medicare cards with new MBIs. Impacted beneficiaries were also offered free credit monitoring and encouraged to monitor their Medicare Summary Notices or Explanation of Benefits and report any unfamiliar claims.
